1 What are the forms and documents to be signed or filled at the time of applying for an insurance policy?

You need to fill and sign the Proposal form, Illustration copy and the Application Verification Form (AVF).

The Proposal Form consists of the Plan details and Personal details of the Life to be Assured, Nominee, Beneficiary and Appointee (where applicable).

The Illustration copy contains the basic policy details such as the Policy term, Frequency, Plan name and assumed returns on investment at various stages of the policy including the effect of charges (applicable only for Unit Linked policies).

The Most Important Document (MID) /Application Verification Form (AVF) is a one page Declaration Form which you need to fill, that will serve as an understanding of the key features of your policy.

Address Proof, Identity Proof, Age Proof and Income Proof should be also submitted along with the documents stated above.

As per regulatory changes, it is mandatory to submit PAN/Form 60 (if PAN is not available). In case you have already submitted the PAN, you may ignore this instruction.

2 What is my Policy Number and Client ID?

Your policy number consists of eight digits and can be found on the first page of your policy document. Remember to quote the policy number in every correspondence, as it helps us access your records faster and better.

The Client ID consists of eight digits, which is located at the right hand side of your renewal premium receipt. This Client ID remains unique, even if you have more than one policy with us.
